Botulinum Toxin/ Neurotoxin

Botulinum toxin is one of the most versatile treatments available in aesthetic medicine today. At The Clinic, it is used to address far more than lines and wrinkles. For many patients, it offers genuine relief from conditions that affect daily life, including teeth grinding, excessive sweating, persistent headaches and gummy smiles. For others, it is a way to soften the signs of repeated expression and restore a sense of ease to the face.

Dr Zamani is one of a handful of UK based Doctor treating Dry Eye Syndrome and Rosacea affecting the eye area with BroadBand Light (BBL) technology. BBL works by safely reducing inflammation in the Meibomian Glands of the eyelids; Meibomian Gland Dysfunction causes approximately 50% of dry eyes.  BBL, a form of IPL, helps to improve tear quality and improve patient reported symptoms.

Patients with ocular rosacea have dilated blood vessels known as telangiectasia along the eyelid margin, which contributes to increased eyelid inflammation and can cause meibomian gland dysfunction.  BBL causes photocoagulation of these abnormal blood vessels, helps open the meibomian gland, and liquifies the oil within the gland.

WHAT TO EXPECT:
Treatment requires protective contact lens placement.  There may be mild swelling, redness or crusting of the area for 5-7 days after treatment.   Most patients notice an improvement within days of having the first treatment.  3-4 treatments are recommended at 2-3 week intervals with subsequent yearly touch ups.

WHAT ARE THE BENEFITS:
BBL is an excellent treatment for dry eyes syndrome and functions to decrease inflammation of the meibomian glands and improve eye comfort.

We offer a range of different Botulinum Toxin A treatments including Botox® Cosmetic, Dysport®, Azzalure®  Bocouture®  and Xeomin®, all of which use similar neurotoxins to temporarily give skin a smoother, more youthful appearance.

WHAT TO EXPECT:

Your first appointment begins with a thorough consultation. Dr Zamani will take the time to understand what is bringing you in, assess the areas of concern and talk you through the most appropriate course of treatment. Nothing is rushed, and no treatment is recommended unless it is genuinely the right option for you.

The treatment itself takes around 20 minutes. A series of small injections are administered using very fine needles, and most patients find the experience straightforward and well tolerated. There is no need for anaesthetic, and you can return to your normal day immediately afterwards. Some mild bruising or swelling at the injection site is possible, but this typically settles within 24 hours. Results become visible within two to eight days and last between three and six months.

Dr Zamani works with a range of botulinum toxin formulations, including Botox Cosmetic, Dysport, Azzalure, Bocouture and Xeomin. The choice of product is always made on the basis of what is most appropriate for you.

WHAT ARE THE BENEFITS:

Botox is a very effective option to soften fine lines and wrinkles without resorting to more invasive. Botox can also be used in conjunction with other facial aesthetic procedures such as dermal fillers or laser treatment to optimise results. Not just reserved for lines and wrinkles, Botox has many uses and can also help to reduce gummy smiles, minimise teeth grinding, aid in the treatment of headaches and treat excessive sweating (armpits, hairline, hands, feet, groin, and menopausal sweats).

WHAT TO EXPECT?

By making an incision along the natural eyelid skin fold, it is possible to remove skin and fat and leave only a fine, discrete scar along the natural skin crease of the eye. Excess upper eyelid skin can lead to functional problems in the eye; causing a loss of peripheral vision, especially in the outer and upper parts of the visual field. Initial bruising and swelling resolve in as little as one to two weeks, however, the final outcome of the surgery may not be seen for several weeks.

On the day of surgery, the amount of skin to be removed is carefully marked in an upright position and then rechecked with the patient reclined to ensure that the eyes will close without difficulty after surgery. A local anaesthetic is injected under the skin and the incisions are placed within the eyelid. The procedure generally takes 30-60 minutes. Fat pads that are normally present in the eyelids can be trimmed or repositioned to contour the eyelid for a youthful appearance. Non-dissolvable sutures are placed under the skin to minimize scarring, and bandages may be placed to secure the sutures. These sutures will be removed 1 week later.


Immediately after surgery, you may notice your vision is blurry due to the antibiotic ointment that has been applied during surgery. You will be asked to use this ointment at bedtime in the eye and 
on the wound. Artificial tears will be prescribed to prevent dryness in the postoperative period.
The degree of swelling and potential bruising differs from patient to patient. We will give you cold compresses to use for 10 minutes every hour while awake for the first 48 hours to minimize bruising and swelling. It is encouraged to sleep with 2-3 pillows to help prevent fluid accumulation
and swelling.

Regular daily activities can resume the day of or after surgery, but exercise and strenuous activity should be avoided until after the first postoperative visit. I recommend no driving until after the sutures have been removed. Patients return to 1 week after surgery for examination and suture removal. Eye make up can be resumed 7-10 days after surgery. It is normal to have feelings of numbness along the eyelash margin, tightness of the eyelids and mild dryness temporarily. Most patients do not experience any pain after the surgery.

It is important to keep the eyes dry for the first 48 hours. Hair can be washed. The eyes can be gently washed from days 3-7 and once the sutures are removed at one week, normal washing routines are initiated.

Serious complications after upper eyelid blepharoplasty are rare. However, there are potential
risks and complications as with any surgery: infection, bleeding, and poor wound healing. In an experienced and conservative surgeon’s hands, complications such as the inability to close the eyes or vision impairment are extremely unlikely. A detailed discussion of all possible risks and complications is made at the time of operative consultations and detailed with the written information summarizing discussions.

Blepharoplasty is designed to improve the appearance of the eyelids and most patients do not feel the need to have a second surgery. However, the natural ageing process will continue and results will vary between patients. Blepharoplasty results last between 7-15 years. Blepharoplasty can be enhanced by other surgical and non-surgical procedures done at the same time.

What are the benefits of Upper Eyelid Blepharoplasty?

Upper eyelid blepharoplasty restores the field of vision, as well as the natural youthful beauty of the eye. By removing excess skin and fat, the eyelid will appear smooth and better defined, giving a more youthful appearance while often improving peripheral vision and eyelid fatigue. External skin resurfacing with a chemical peel or laser resurfacing treatment may be possible at the same time to improve the appearance of crepey skin.

What are neurotoxins used for in medical aesthetics?

Botulinum toxin is used to relax the muscles behind dynamic wrinkles, the lines caused by repeated facial movement like frowning or squinting. Over time, as skin loses elasticity, those lines start showing even when your face is at rest, and neurotoxin softens them for a smoother, more refreshed look. Beyond wrinkles, it's also used for a gummy smile, teeth grinding, headaches, masseter hypertrophy, neck bands and excessive sweating.

What is the "best" neurotoxin injectable?

There isn't one single "best" option, it really comes down to your goals, how your muscles move, and what the injector recommends after assessing your face. At the clinic, treatment starts with a personalised consultation to work out muscle balance and pick the right product and dose for you.

Which brands do you offer (Botox, Dysport, Xeomin, etc.)?

What are the side effects and safety considerations?

How much downtime is there after Botox/neurotoxin?

Very little. Most people go straight back to their day. Some mild bruising can happen, but there's no significant recovery period needed.

How long is the appointment, does it hurt?

Sticking to aftercare in the first 24 hours (staying upright, avoiding touching the area, skipping vigorous exercise and alcohol) also helps the product settle properly where it's meant to work.

Botox vs fillers: what's the difference and which is right for you?

They solve different problems. Botox relaxes the muscles behind dynamic wrinkles, the lines caused by repeated movement like frowning or squinting, so it's the right choice for expression lines on the forehead, between the brows, or around the eyes. Fillers add volume, so they're better suited to areas that have lost structure or fullness, cheeks, lips, jawline, under-eyes, or deeper static lines that show even when your face is at rest. A lot of patients actually benefit from both together as part of one treatment plan, since Botox alone can't tighten skin or restore lost volume, and fillers alone won't stop movement-related lines from forming. Which one you need (or whether it's both) really comes down to what's assessed at consultation.

Botox treatment: what actually happens during the procedure?

It starts with a consultation where the doctor checks your facial movement in both a relaxed and expressive state, goes over your medical history, and discusses what you're hoping to achieve. From there, the treatment itself is quick, fine needles are used for injections, so there's minimal discomfort, and no numbing cream or local anaesthetic is required.
